Form F4 - Application for Mains & Services (Requisition)

This form should be completed once a decision to requisition mains and/or services from Bristol Water has been taken. Developers requiring budget information to allow them to choose between the requisition and self-lay routes should, instead, completed a Preliminary Mains Enquiry form (F2). Only the owner or occupier of a site can requisition mains. Consultants acting on their behalf cannot do so. Either the owner or occupier must e-sign this application.
